HICMR Assessment and Reporting Program (HARP)

This is a restricted site allowing access by the facility Director of Nursing however named, and one other nominated representative only. Access to the specific materials and documents available on this site is by contractual arrangement only.

Risk Assessment (RA) Reports & Action Plans

HICMR ICCs conduct regular Risk Assessments in all relevant practice/specialty areas using their unique web based RA Tools, to assess and measure compliance with relevant current standards, guidelines and Policies.

Sequential RAs, which are conducted ongoing based on score achieved, are available as separate reports by RA Tool. Base/Periodical Risk Assessments, ie. those conducted annually, may be a compiled Report.

RA Reports require confirmation of receipt within 30 days - please refer to the specific RA Report for confirmation process. Action Plans are now also available for internal client use to assist with management and documentation of follow-up action.

The HARP RA Tools were progressively developed from February - December 2007. Therefore, some RAs may not have been made available until 2008. The RA Tools are regularly reviewed and revised as required, and may result in a change of total score for that RA. Action Plans were also developed in July-August 2008, so may not be available prior to this.

RA Graphs

HICMR RA results can be benchmarked by facility and RA Tool, both internally and externally against other similar HICMR facilities.

The most recent score for the RA Tool for that year is represented.

These Phase 1 Graphs are currently being trialed by the clients. Therefore, please check your facility's Graphs as soon as possible, and notify your HICMR ICC if there are any issues you wish to discuss further.

It is anticipated that Phase 2 Graphs will also include benchmarking by State, Owner, type of RA/Specialty Area.
